Аннотация
Хроническая обструктивная болезнь легких (ХОБЛ) является серьезной проблемой как для отечественной, так и для мировой медицины. Это обусловлено неуклонным ростом числа больных. Как показали многочисленные исследования, основная роль в развитии обострений ХОБЛ принадлежит инфекционному фактору. Бактериальная колонизация нижних дыхательных путей распространена у пациентов с ХОБЛ, но роль микробиоты и иммунного ответа макроорганизма на потенциальные патогены изучена недостаточно. При ХОБЛ обнаруживается нарушение состава микробиоты дыхательных путей, что способствует дезадаптивным изменениям иммунной системы легочной ткани и ассоциируется с повышенным уровнем воспаления. В данном обзоре подробно исследуется роль микробиоты в патогенезе ХОБЛ и ее влияние на течение болезни. В связи с этим дополнительно обсуждаются роль вакцинации против пневмококковой инфекции и антибиотикорезистентность респираторных патогенов.
