Аннотация
Бактериальная транслокация считается основным механизмом развития бактериальных инфекций и формирования провоспалительного статуса у пациентов с циррозом печени, в то же время четких критериев для оценки этого явления не существует. Данная статья представляет собой обзор современных публикаций, посвященных проблеме транслокации кишечной микрофлоры. В обзоре рассматриваются изменения кишечного микробиома, местного физиологического барьера, а также особенности врожденного и адаптивного иммунитета, которые обусловливают формирование и прогрессирование цирроза печени. Приведены результаты исследований потенциальных маркеров бактериальной транслокации (С-реактивный белок, прокальцитонин, липополисахарид, пресепсин и др.) у пациентов с циррозом печени и их использования для прогнозирования развития инфекции и летальности. Описаны современные методы, используемые для изучения кишечного микробиома, а также направления будущих исследований.
