Аннотация
Пациенты, которые получают цитотоксическую химиотерапию или которым проводится трансплантация гемопоэтических стволовых клеток, входят в группу риска развития инфекций. Вероятность развития инфекционных осложнений возрастает с увеличением степени и продолжительности нейтропении, при этом наибольший риск отмечается у больных с трансплантацией гемопоэтических стволовых клеток, во время индукции ремиссии при остром лейкозе, а также при других состояниях, сопровождающихся глубокой длительной нейтропенией. Именно у таких пациентов, находящихся в группе высокого риска, рекомендована антимикробная профилактика. Обновлённый вариант рекомендаций, созданных совместно Американским обществом клинической онкологии (ASCO) и Американским обществом по инфекционным болезням (IDSA), содержит информацию по антибактериальной, противогрибковой и противовирусной профилактике у этой категории пациентов. В статью также включён сравнительный анализ других международных документов по данной теме — рекомендаций Национальной всеобщей онкологической сети США (NCCN) по профилактике и терапии инфекций, связанных со злокачественными новообразованиями, и Европейской конференции по инфекциям у пациентов с лейкозами (ECIL) по первичной противогрибковой профилактике у взрослых пациентов с онкогематологическими заболеваниями.
