Basic Principles of the Local Treatment of Wounds and Wound Infection

Abstract

Due to success in antimicrobial therapy of skin and soft tissue infections, current consensus guidelines give less attention to surgical aspects of wound and wound infection treatment. This paper presents evolution of approaches to treatment of wounds over the last decades, as well as alternative approach to determination of wound process phase and to choice of local treatments. Main factors contributing to delayed/poor wound healing and algorithm for determining those factors are also considered. Different types of surgical debridement and their advantages and disadvantages are discussed. Review of modern antiseptics, dressing, and the most common ointments used for the local treatment of wounds in Russia is provided. Some promising options that may be used in the management of patients with wounds are listed. Finally, several clinical examples demonstrate common errors in the wound treatment of wounds.
